Our office accepts most insurance carriers. However, please contact the customer service number located on the back of your card or log onto the carriers website to be sure we are listed as an In Network provider prior to booking an appointment.
We do not accept out of network plans. If your plan requires a co-pay for the visit, it will be collected at the time of the visit. Please have your card with you at each visit so we are sure to file the claim with the correct carrier. If you are not insured. you will be responsible for the entire charge at the time of service.
