Billing and Insurance

It is in your best interest to know and understand your insurance policy, benefits, deductible, co-payments and authorization requirements before you seek medical services. If you have questions as to what is covered by your insurance we recommend that you call your insurance company directly. We accept numerous health plans and cannot know all of the details of your policy. You are responsible for providing us with all current insurance information. Many health plans restrict the hospital, lab or radiology facilities that you may use. Some may also require authorization prior to a referral. It is your responsibility to know the hospital, lab, and radiology facility used by your insurance plan. Failure to provide us with this information may result in your insurance company’s denial to pay for services which will leave you responsible for the payment.

Co-payments and payment towards your deductible are collected at the time of your visit. We accept Cash and some Credit Cards. There is a $50.00 returned check fee. Please bring your insurance card to every visit. If you do not have proof of insurance at the time of your visit you will be expected to pay for the visit at that time.

It is very important that you contact your insurance company immediately to notify them of any sentinel events including the birth of a child, marriage, or job changes. In the case of a newborn, you usually have 15 – 30 days following delivery to add your child to your health insurance policy.
